Doel Richtlijnen voor de behandeling van acute bronchitis en COPD adviseren om terughoudend te zijn met antibioticagebruik. Patiënten met relevante comorbiditeit ontwikkelen echter vaker complicaties, waardoor antibiotica mogelijk wel zinvol kunnen zijn. Wij hebben onderzocht of huisartsen vaker antibiotica voorschrijven aan oudere patiënten met potentieel risicovolle aandoeningen, die op het spreekuur kwamen met acute bronchitis of exacerbaties van COPD.
Methode Met behulp van het Huisartsen Netwerk Utrecht (HNU) selecteerden we 2643 episodes van patiënten van 65 jaar of ouder, bij wie de huisarts de diagnose acute bronchitis of COPD-exacerbatie had gesteld. Met multivariabele regressieanalyse bepaalden we onafhankelijke voorspellers van antibioticavoorschriften.
Resultaten De huisartsen schreven vaak antibiotica voor bij acute bronchitis (84%) en bij COPD-exacerbaties (53%). We vonden alleen een relatie tussen leeftijd en antibioticavoorschriften als de diagnose acute bronchitis was gesteld (OR 1,03, 95%-BI 1,003-1,048), terwijl antibiotica in het geval van COPD-exacerbaties vaker werden voorgeschreven aan mannelijke patiënten (OR 1,3, 95%-BI 1,0-1,5), patiënten met diabetes mellitus (OR 1,7, 95%-BI 1,1-2,4) en/of aan patiënten met hartfalen (OR 1,3, 95%-BI 1,0-1,7).
Conclusie Bijna alle ouderen met acute bronchitis en meer dan de helft van de patiënten met COPD-exacerbaties krijgen antibiotica voorgeschreven. Het voorschrijfgedrag kan verbeterd worden als huisartsen meer rekening houden met de aan- of afwezigheid van comorbiditeit.
