Samenvatting

Introductie:

In Nederland ontwikkelen jaarlijks naar schatting 1100 patiënten koorts na een prostaatbiopsie. Deze studie wil advies geven voor antibiotische behandeling van koorts na het nemen van transrectale prostaatbiopten of na transrectale goudmarkerimplantatie op basis van de gevonden resistentiepatronen.

Materiaal:

Patiënten bij wie een prostaatbiopsie of goudmarkerimplantatie is uitgevoerd vanaf 2008 tot en met 2011 werden onderzocht. Eén uur voor de ingreep werd ciprofloxacine ingenomen. Patiënten werden geïnstrueerd zich te melden bij koorts boven 38,5 °C. Patiënten met een positieve urine- en/of bloedkweek na de ingreep werden geïncludeerd.

Resultaten:

Er werden 1435 biopsieën of goudmarkerimplantaties verricht. Bij 40 verrichtingen (3,0%) werd een positieve kweek (18 bloed- en 30 urinekweken) gevonden met 27 maal (2,0%) een urosepsis. Resistentie inclusief intermediaire gevoeligheid voor ciprofloxacine, amoxicilline/clavulaanzuur en cotrimoxazol zijn respectievelijk 52%, 39% en 55%. Voor cefuroxim en tobramycine is dit respectievelijk 16% en 20%.

Conclusie:

Patiënten met koorts na het nemen van prostaatbiopten of na goudmarkerimplantatie zouden behandeld moeten worden met intraveneuze antibiotica.
