Naast de specialist speelt de huisarts een grote rol in het begeleiden van patiënten die behandeld worden voor prostaatcarcinoom. De standaard behandelingsopties bij gelokaliseerd prostaatcarcinoom zijn, naast actief volgen, radicale prostatectomie en radiotherapie. Dit zijn ingrijpende behandelingen met lastige bijwerkingen. Nu de incidentie van prostaatkanker toeneemt, stijgt ook de vraag naar minder invasieve behandelingsmethoden met hopelijk minder bijwerkingen. Dit nascholingsartikel gaat in op de uitkomsten en bijwerkingen van de standaardbehandelingen, en op twee relatief nieuwe ontikkelingen: cryochirurgie enhigh intensity focused ultrasound(HIFU). Deze zogeheten focale therapieën zijn sterk in opkomst. Juist omdat ze nieuw zijn en volop in ontwikkeling, is over hun effectiviteit en bijwerkingen nog weinig met zekerheid te zeggen. Zowel cryochirurgie als HIFU hebben redelijk wat bijwerkingen – met name urineretentie en erectiele disfunctie – en de resultaten op lange termijn zijn onzeker. Daarom zijn ze voorlopig nog geen serieus alternatief voor de standaardbehandelingen.
