Samenvatting

Het beroep van bekkenfysiotherapeut is volop in beweging en er is steeds meer behoefte aan wetenschappelijke onderbouwing en optimalisering van behandeltechnieken zowel qua techniek als uitvoering. Nationaal en internationaal wordt bekkenfysiotherapie erkend als een eerste behandeloptie bij mictieklachten met een ‘Level A, Grade I evidence’. Deze positionering is alleen blijvend te rechtvaardigen als voldoende aandacht wordt besteed aan wetenschappelijke onderbouwing van de behandeling als zodanig en van de behandelresultaten.
De bekkenfysiotherapeut maakt gebruik van een gevalideerde vragenlijst om een totaalbeeld te krijgen van de klachten van de patiënt. Deze vragenlijst kan ook als onderzoeksinstrument worden gebruikt. Daarna volgt een onderzoek naar de functie van de bekkenbodem. Dit onderzoek bestaat uit een kwalitatief en een kwantitatief onderzoek. Verschillende methoden, elk met eigen voor- en nadelen, worden gebruikt voor het beoordelen van de functie van de bekkenbodem: digitale palpatie, manometrie, EMG-registratie (biofeedback), echografie, en tegenwoordig ook MRI.
Uit het bekkenfysiotherapeutische onderzoek volgt de bekkenfysiotherapeutische diagnose.
Bekkenfysiotherapie met behulp van onder andere EMG-registratie is effectief bij de behandeling van bekkenbodemdisfuncties. Voor het optimaliseren van de behandelresultaten zijn echter meer randomized controlled trials (RCT’s) nodig, met gestandaardiseerde behandelprotocollen, met voldoende patiëntenaantallen en controlegroepen. De behandelresultaten in deze studies moeten verzameld worden met gevalideerde meetinstrumenten (vragenlijsten, EMG-meetinstrument), met het doel meer uniformiteit te verkrijgen in de diagnose en behandeling van bekkenbodemdisfuncties. Door het gebruik van gevalideerde meetinstrumenten is het mogelijk om in de toekomst verschillende studies met elkaar te vergelijken, wat op dit moment onmogelijk is.
