In dit case-report beschrijven wij de bevindingen bij bronchoscopie bij een jongetje van 1 jaar met een agenesie van de rechterlong. Er werd driemaal door hetzelfde team een scopie verricht en wel op de leeftijd van 1, 4 en 20 jaar. Er waren steeds meerdere afwijkingen tegelijkertijd. De toegepaste techniek is gebaseerd op een combinatie van algehele en lokale anesthesie, waarbij spontane respiratie wordt gehandhaafd. Door alleen gebruik te maken van een Hopkins-optiek is er sprake van een atraumatische procedure. Op deze manier kunnen zowel anatomische als functionele afwijkingen tegelijkertijd worden gediagnosticeerd. Fotografische verslaglegging vormt een essentieel onderdeel van de werkwijze. Dit maakt evaluatie van de bevindingen en intercollegiaal overleg mogelijk.
Summary
In this case report we describe the bronchoscopic findings in a one year old boy with a right sided pulmonary agenesis. The procedure was performed two times thereafter at 4 and 20 years of age by the same team. The pathology of the airways was varied and different every time. The bronchoscopic technique employed is based on a combination of general and local anaesthesia, combined with maintenance of spontaneous respiration. The use of a rigid Hopkins optic only allows a very atraumatic endoscopic examination of the entire airway. In this way anatomical as well as functional abnormalities can be detected and studied at the same time. Photographic techniques should be used with the procedure. This facilitates evaluation of the findings and assists in discussion with colleagues.
