Samenvatting

Traumatisch hoofd-/hersenletsel (THL) is een aandoening met directe en indirecte neurologische consequenties. Eenmalig en recidiverend THL geeft een verhoogd risico op het ontwikkelen van een neurodegeneratieve aandoening zoals de ziekte van Alzheimer, ziekte van Parkinson, frontotemporale dementie, amyotrofische laterale sclerose (ALS) en specifiek voor recidiverend THL chronische traumatische encefalopathie (CTE). CTE wordt klinisch gekenmerkt door een spectrum van cognitieve, psychiatrische en motorische symptomen en specifieke histopathologische afwijkingen door ophoping van hypergefosforyleerd tau-eiwit en de aanwezigheid van TAR-DNA-bindend eiwit (TDP-43). Het risico op het ontwikkelen van een van deze ziekten na een THL is wisselend. De diagnostiek van neurodegeneratieve ziektes met behulp van serumbiomarkers en radiologische biomarkers is in toenemende mate mogelijk. Dit biedt mogelijkheden om in de toekomst de associatie tussen de verschillende neurodegeneratieve aandoeningen en THL verder te onderzoeken.
