Toegevoegde waarde van continue subcutane glucosemeting bij behandeling van kinderen met diabetes mellitus type 1

Samenvatting

Continue subcutane glucosemeting (CGM) is een nieuwe technische toepassing die steeds vaker wordt gebruikt in de behandeling en begeleiding van kinderen met diabetes mellitus type 1. In dit artikel bespreken wij onze ervaringen met het gebruik van de Medtronic MiniMed, een continue glucosesensor van het zogenoemde holter-type. Met dit systeem kunnen gedurende een periode van 72 uur continu glucosemetingen worden opgeslagen, die vervolgens kunnen worden uitgelezen en geanalyseerd. De procedure en indicatiemogelijkheden bij kinderen met diabetes mellitus type 1 worden besproken. In onze ervaring leidden aanpassingsadviezen gebaseerd op glucosesensormetingen tot een verbetering van de metabole instelling, zich uitend in een dalende GlyHb van 8,4 (SD 1,3) bij de start van de sensormeting naar 8,0 (SD 1,2) na zes maanden (p < 0,01, ANOVA: F = 2,4). Het gecombineerd bijhouden van een eetdagboek geeft bruikbare, additionele informatie. Het effect van CGM op een verbeterende metabole instelling is in de literatuur vooralsnog niet eenduidig en dient verder onderzocht te worden. Toekomstige ontwikkelingen naar een gebruik van realtime continue glucosemonitoring wordt kort besproken. Kennis van deze nieuwe technologische ontwikkelingen is onontbeerlijk voor de dagelijkse praktijk van de kinderdiabeteszorg.
