8. Afwijkingen in het hoofd-halsgebied

Samenvatting

Gezien het veelvuldig voorkomen van letsels in het hoofd-halsgebied moet bij alle kinderen bij wie een vermoeden bestaat van fysieke kindermishandeling, verwaarlozing of seksueel misbruik een gedegen onderzoek plaatsvinden van dit gebied, inclusief de mond- en keelholte. De meest voorkomende letsels in het gelaat zijn onderhuidse bloeduitstortingen. Traumatische afwijkingen in en rond de mond kunnen bij lichamelijke mishandeling worden veroorzaakt met voorwerpen zoals lepels en vorken of een fles tijdens geforceerde voeding en met handen/vingers of door hete vloeistoffen of etsende stoffen. Vanwege het ontbreken van specifieke letselkenmerken is differentiatie tussen accidentele en toegebrachte letsels alleen mogelijk door te beoordelen of het verhaal over het ontstaan, inclusief de timing en het mechanisme, consistent is met de letselkenmerken en het ontwikkelingsniveau van het kind. Er bestaat op dit moment onvoldoende evidence om een gescheurd frenulum te beschouwen als kenmerkend voor kindermishandeling indien de afwijking geïsoleerd voorkomt.
