Samenvatting

Volgens de klassieke economische theorie betekenen meer opbrengsten dan kosten een positieve prikkel voor implementatie. De vertaling van deze economische theorie naar de gezondheidszorg is echter niet eenvoudig. Uit observationeel onderzoek blijkt veelal dat hoogte van vergoedingen voor zorgverleners en aantal verrichtingen positief met elkaar samenhangen. Wel moet rekening worden gehouden met neveneffecten. Meer kosten of financieel risico voor gebruikers van zorg leiden in het algemeen tot lager zorggebruik. Er zijn aanwijzingen dat dit geldt voor zowel effectieve als niet-effectieve zorg. Het bevorderen van transparantie van kwaliteit (via prestatie-indicatoren) kan bijdragen aan de marktwerking. Individuele gebruikers van zorg maken hiervan weinig gebruik. Wet- en regelgeving kan bijdragen aan betere kwaliteit van zorg, maar het onderzoek hiernaar staat nog in de kinderschoenen. Wel is een raamwerk beschikbaar voor het vormgeven van landelijk implementatiebeleid waarin zowel makers als gebruikers van kennis bepaalde taken krijgen.
