De MiniArc sling voor stress urineincontinentie bij vrouwen; klinische resultaten na 1 jaar follow-up

Samenvatting

Introductie: Urine-incontinentie wordt gedefinieerd als het ongewild verlies van urine en is geassocieerd met een daling in de kwaliteit van leven (KvL). De schatting is dat tussen de 10 en 40% van de vrouwen van 15-64 jaar te maken zal krijgen met urine-incontinentie en dat dit percentage bij vrouwen ouder dan 65 jaar zelfs nog hoger ligt. Een operatie door middel van een mid-urethrale sling is een geaccepteerde en bewezen effectieve methode om vrouwen van hun stress urine-incontinentie (SUI) te genezen. In navolging van de tension-free vaginal tape (TVT), de tension-free vaginal tape-obturator (TVT-O) en de trans obturator tape (TOT) zijn de ‘mini-slings’ zoals de MiniArc (2007 AMS) ontwikkeld. De TVT, TVT-O en TOT laten alle succespercentages zien tussen de 84 en 100% na een minimale follow-up van 1 jaar. Het doel van deze studie was het evalueren van de effecten van de MiniArc 1 jaar postoperatief, met aandacht voor zowel de continentie als de kwaliteit van leven.
Materiaal en methoden: 77 patiënten, geopereerd tussen maart 2008 en november 2009, zijn in deze studie geëvalueerd. Gepresenteerd worden de data 1 jaar postoperatief. Preoperatief hadden alle vrouwen last van geobjectiveerde SUI. Na 1 jaar was de respons 74%. De patiënten zijn geëvalueerd met de vragenlijst voor urine-incontinentie van de Nederlandse Vereniging voor Obstetrie en Gynaecologie (NVOG), die is samengesteld uit de EuroQol-5 Dimensies (EQ-5D), de Patient Global Impression of Improvement (PGI-I), de Incontinence Impact Questionnaire (IIQ), de Urinary Distress Inventory (UDI), de Prolapse/Urinary Incontinence Sexual Questionnaire, korte vorm (PIS-Q SF) en de Defecation Distress Inventory (DDI).
Resultaten: Eén jaar postoperatief gaf 68% van de patiënten aan dat de SUI verbeterd was, terwijl slechts 44% beaamde compleet droog te zijn. De UDI (stress) was in alle subcategorieën significant verbeterd na 1 jaar. De IIQ (alledaags functioneren) liet na 1 jaar een significante verbetering zien in 3 van de 5 subcategorieën.
Conclusie: De follow-up na 1 jaar suggereert dat de MiniArc minder effectief is bij de behandeling van SUI dan de gang-bare slingprocedures.
