Abstract

Afhankelijkheid van opiaten komt voort uit en leidt tot een complex van onderling samenhangende problemen. In Nederland zijn er ongeveer 26.000 patiënten met afhankelijkheid van opiaten. In het algemeen zijn eenvoudige interventies gericht op één probleem uit dit complex gedoemd te mislukken bij deze patiëntengroep. In de behandeling wordt daarom niet alleen aandacht besteed aan lichamelijk en geestelijk functioneren, maar wordt ook gekeken naar arbeidsomstandigheden, opleiding en inkomen, en relationele aspecten. Een gefaseerd behandelingsplan voor complexe problematiek draagt vaak het karakter van een langdurig behandelingstraject.

CRA: behandelingsmethode waarbij getracht wordt op zoveel mogelijke terreinen steun en behandeling te bieden. Bijvoorbeeld door het innemen van medicatie onder supervisie van een coach die zelf niet verslaafd is, het leren omgaan met zucht naar opiaten, het volgen van een opleiding of het leren wennen aan regulier werk. Kern is dat ander gedrag dan gebruik van psychoxatieve stoffen zoveel mogelijk beloond wordt en dat de patiënt leert terugval te voorkomen of te beperken.
