Samenvatting

Plasklachten na kankerbehandelingen komen frequent voor, maar worden niet altijd ter sprake gebracht. Onderscheid maken in klachten gerelateerd aan opslag en klachten gerelateerd aan lozing helpt voor het begrip van de pathofysiologie en de in te stellen behandeling. Een radiatiecystitis is een veelvoorkomende diagnose na radiotherapie op de bekkenorganen. Het gaat vaak gepaard met opslag-gerelateerde urinewegsymptomen en kan gepaard gaan met hematurie. Blaasspoelingen bij blaaskanker, urineverlies na prostaatkankerchirurgie en fistels tussen de urinewegen en de vagina zijn andere oorzaken voor opslag-gerelateerde plasklachten. Naast niet-medicamenteuze adviezen kan een medicamenteuze behandeling met spasmolytica geprobeerd worden. Specifieke en oorzakelijke behandelingen zijn beschikbaar in de tweede lijn. Bij lozing-gerelateerde klachten, bijvoorbeeld na chirurgie van de bekkenorganen is het van belang overloopincontinentie uit te sluiten en zo nodig de blaaslediging te verzekeren door katheterisatie.
