2. De behandeling van slapeloosheid zonder pillen

Samenvatting

Bij slapeloosheid wordt een onderscheid gemaakt tussen kortdurende en langdurige insomnie. De NHG-standaard spreekt al van langdurige insomnie na drie weken. De algemene definitie van langdurige slapeloosheid (of insomnie) volgens de ‘International Classification of Sleep Disorders’ (American Academy of Sleep Medicine, 2014) is: minimaal drie nachten per week problemen met in- of doorslapen of te vroeg wakker worden gedurende minimaal drie maanden. Een voorwaarde hierbij is dat er voldoende mogelijkheid is tot slapen (voldoende tijd, veilig, donker, stil en comfortabel). Overdag zijn er klachten zoals vermoeidheid, moeite met concentreren of onthouden, prikkelbaarheid, somberheid of slaperigheid. In de DSM-5 is de term primaire insomnie vervangen door de diagnose ‘insomnie-stoornis’ om het onderscheid primair/secundair te vermijden als de slaapstoornis gelijktijdig voorkomt met andere stoornissen. Behandeling van de slapeloosheid vormt een apart behandeldoel, ook al is er bijkomende medische of psychiatrische problematiek aanwezig (Harvey, Cognitive Behaviour Therapy 38:35–42, 2009).
