Menselijke fouten kunnen worden onderverdeeld in twee categorieën. Sommige fouten (vergissingen of mistakes) worden begaan door een gebrek aan kennis over de correcte actie. Dit kan bijvoorbeeld gebeuren als iemand een situatie verkeerd interpreteert en vervolgens handelt op basis van die verkeerde interpretatie. Meestal worden mensen zich pas bewust van dit soort fouten door een signaal uit de omgeving. Zulke externe feedback vormt vaak de basis voor leren. Een ander type fouten (slips, een goede Nederlandse vertaling ontbreekt) wordt gekenmerkt door het uitvoeren van een verkeerde actie ondanks de aanwezigheid van de juiste kennis en intentie. …
