6. Fracturen op de kinderleeftijd: accidenteel of toegebracht letsel?

Samenvatting

Hoewel kinderen vaak iets breken, moet in geval van een fractuur bij een niet-mobiel kind altijd aan kindermishandeling gedacht worden. Bij de verdenking op kindermishandeling onder de leeftijd van 2 jaar wordt een skeletstatus vervaardigd om fracturen op te sporen. Bij kinderen jonger dan 1 jaar wordt daarnaast standaard een CT schedel vervaardigd. Fracturen met een hoge specificiteit voor kindermishandeling zijn posterieure ribfracturen en metafysaire hoekfracturen. In dit hoofdstuk worden de verschillende radiologische technieken besproken en het traumamechanisme dat het letsel veroorzaakt. Het is van belang dat de beelden beoordeeld worden door een (kinder-)radioloog met voldoende ervaring binnen deze problematiek. Indien nodig dient een meer ervaren kinderradioloog geraadpleegd te worden, deze zijn aangesloten bij het Landelijk Expertise Centrum Kindermishandeling (LECK).
